Who we are

This website and your medical records

This policy covers this website only: what it collects when you visit it or use one of its forms. It does not describe how we handle your clinical records. Health information created when you are a patient of ours is protected by the federal HIPAA Privacy Rule and is described in our Notice of Privacy Practices, which prevails if the two ever differ.

What information we collect

We only collect information you choose to send us. This website does not require an account, and we do not ask visitors to enter payment details here.

Contact form. When you use the contact form we collect the name, email address and message you enter.

Employment application form. When you apply for a role we collect the position applied for, your first and last name, email address, optional phone number, availability, optional desired start date, optional years of experience, your self-reported IV experience level, and the summary you write.

Shop pickup requests. When you ask us to set items aside from the shop, we collect the items and quantities you chose, your name, your email address, an optional phone number and any optional notes you add. The shop does not take payment on this website.

Information collected automatically. Like any website, our hosting provider (Vercel) receives standard request information — your IP address, browser and device type, the page requested and the time — in order to deliver the site and keep it secure. Page content and images are delivered from our content platform (Sanity). We do not use this information to identify you or to build a profile of you.

Please do not send us medical history, diagnoses, or other health details through the website forms. If you need to discuss anything clinical, call us or raise it at your appointment so it can be handled through our clinical records rather than a website form.

How form submissions are handled

The contact form, the employment application and the shop pickup request are all delivered to us by Formspree, a third-party form-processing service. Your submission passes through and is stored on Formspree’s systems in order to reach our inbox, and is therefore also subject to Formspree’s own privacy practices. We use the information solely to respond to your enquiry or to consider your application.

Booking and third-party services

Appointment booking links on this site take you to Jane, our external scheduling provider. Anything you enter there is collected by Jane under its own privacy policy and terms, not by this website. Links to Instagram, Facebook and YouTube likewise take you to services governed by their own policies.

Our Locations page shows a Google map. Loading it sends your IP address and browser details to Google, which may set its own cookies under its own privacy policy. Some images are served from third-party image hosts, and requests to those hosts likewise expose your IP address to them as a normal part of loading the page. Our website fonts are served from this site itself, not from a third party.

The links to our mobile app take you to the Apple App Store or Google Play, which are run by Apple and Google under their own terms and privacy policies. What the app itself collects is described by the app and its store listing, not by this website.

Cookies and analytics

This website does not set advertising cookies, does not run third-party analytics or tracking pixels, and does not build visitor profiles. Some interface preferences, and the contents of your shop cart, may be stored locally in your own browser; that data stays on your device and is not transmitted to us until you choose to send a pickup request. You can clear it at any time in your browser settings.

The embedded Google map and links to other services (Jane, Instagram, Facebook, YouTube, the App Store and Google Play) are run by those companies and may set their own cookies once you use them; those are governed by their policies, not ours. Because we do not track visitors across sites, we do not respond differently to “Do Not Track” or Global Privacy Control signals.

If we add website analytics in future, this section will be updated to name the provider and describe what is measured before that tracking begins.

Artificial intelligence

We do not use artificial intelligence to make decisions about your care. Every clinical judgement — whether a treatment is appropriate for you, what is in your infusion, and how you are monitored — is made by a licensed clinician.

This website has no AI features. There is no chatbot, no automated intake screening, and no system that profiles you or decides anything about you automatically. The pages you see are the same for every visitor.

We do not feed the information you send us into AI systems, and we do not use it to train AI models. We cannot speak for the internal practices of the third-party providers named above; their own privacy policies govern how they operate their platforms.

If we ever adopt an AI tool that touches your information or your care, this section will be updated to say what it is and what it does before that tool is put into use.

We do not sell your information

We do not sell, rent or trade personal information. We do not share it for advertising purposes. We share it only with the service providers described above who help us operate the site, or where we are legally required to do so.

How long we keep information

Contact messages are kept only as long as needed to handle your enquiry and our ordinary business records. Job applications are retained for our recruitment records. You may ask us to delete a submission at any time.

How we protect information

Pages and forms on this site are transmitted over encrypted (HTTPS) connections, and we limit who can read what you send us to the people who need it to respond. No method of transmission or storage is completely secure, which is another reason not to send health details through website forms. If a breach affecting your personal information occurs, we will notify you and the authorities as the law requires.

This website is operated from the United States, and the providers named above may process information on servers in the United States or elsewhere.

Your choices

You can ask us what information we hold about you from this website, ask us to correct it, or ask us to delete it. Email yellowstoneiv@gmail.com and we will respond within a reasonable period. Depending on where you live, you may have additional rights under applicable state or national privacy law, such as the right to know, correct or delete personal information. We do not sell personal information or share it for cross-context behavioural advertising, so there is nothing to opt out of. We will not treat you differently for exercising a privacy right.

Children

This website is intended for adults. We do not knowingly collect information from children through it. If you believe a child has submitted information, contact us and we will remove it. Applications to work with us are only accepted from people who are legally old enough to work.

Submitting a form on this site does not create a doctor–patient relationship, and nothing you send through it should be treated as a request for urgent care. In an emergency, call 911.
